Immunizations
We are a vaccinating practice. We understand that many parents have questions about vaccine safety. That’s why we provide written evidence-based information with frequently asked questions about vaccines. We do require that our newborn patients start their vaccine series starting at two months, and are happy to provide answers to vaccine questions and our written material so that parents are comfortable with vaccinating. If you have further questions about vaccinating your children, and want to know if our office would be a good fit for your family, please call our office. We also do not follow alternative or split vaccine schedules. See the Immunization document above which outlines why we follow the CDC schedule.
Appointments
Please arrive for your appointments on time. Patients who arrive more than 15 minutes late may be asked to reschedule. New patients will be asked to arrive 15 minutes early to fill out necessary paperwork and check insurance eligibility. We encourage families to fill out certain paperwork online through our patient portal. A portal pin number can be obtained by calling our office.
New patients who miss more than two appointments or established patients who miss more than three appointments without proper notification will be dismissed. We require 24-hour notice for canceling any appointments. There will be a $25 charge for missed sick and well appointments. Some missed appointments which are booked with extended time like nutrition, mental health or behavior visits will be charged $50.
Patient Paperwork and Policies
If you are transferring from another practice, we ask that you bring a copy of your child’s medical records prior to your first appointment so that we may have time to become familiar with your child’s medical history. You can also ask us for a release of records (or print above) and we can request records for you. You can download the patient paperwork above or on your portal and fill it out prior to your first appointment This includes demographics, HIPAA and financial policies. Please click above to see our policies and download your “New Patient Packet”.
Please allow at least 48 hours for us to complete forms like school or camp paperwork. There is a $10 fee for each form that is requested outside of an office visit. We may charge $5 per child to copy or transfer medical records.
